It’s time for The Arc of Howard County’s annual survey. Please download your survey and submit by January 30th!

Each year the Monitoring and Advisory Committee asks persons who are important to the individuals who are supported by The Arc to complete a survey. Your name was either suggested by or was taken from the files. 

We want to know how The Arc is doing. The Monitoring and Advisory Committee leadership and the Executive Director will review the surveys. We value your opinions and ask for your candid thoughts whether positive or negative.  An annual report, written in general terms, is made to The Arc Howard Board of Directors noting reported strengths and weaknesses of our programs.  This report is based on these surveys and on visits made by committee monitors with the individuals in The Arc programs. The report is presented in the spring to the board in order that any adjustments in services can be included in budget and strategic planning for the coming year.

To complete a survey online you will need Adobe Acrobat Pro:

1. Download your survey from the list in the column to the right.
2. Save the survey to your computer.
3. Open the survey in Adobe Acrobat to fill it out.
4. Save your survey on your computer.
5. Email your saved file back to: tfletcher@archoward.org.
6. Please return all surveys by January 30, 2012.

If you have Acrobat Reader only you will need to:

1. Print your survey.
2. Fill out survey and mail to The Arc of Howard County, 11737 Homewood Road, Ellicott City, MD 21042 or scan and email to tfletcher@archoward.org.

As always, you are welcome to call Carol Beatty or other staff with concerns or questions directly.  Nonetheless, it is helpful to have survey information to complete this part of The Arc’s Quality Assurance Program.  Some questions may not apply to your situation, so please ignore them.  We value your thoughts and concerns and appreciate your time and attention. Please call Tammy Fletcher at 410-730-0638, ex. 264 if you have any questions. We will greatly appreciate having your reply before January 30th.
